Use Microsoft Forms to easily create quizzes, polls and even tests


I have just finished a surprise (and fun) quiz for my lessons tomorrow,  to see what my Grade 4 class remembers about the coding terminology they learned in Term 2. I used Microsoft Forms - simple to use, attractive, easy to send out to my students, and marvelous to mark and give feedback - the form is self-marking! 

What is a Microsoft Form?

Have you ever clicked on the more apps icon in your Office 365 account? if you have your notice that Microsoft forms can be found there. This is a useful web-based application that you could use to create a quiz, survey or test.  

How do you set it up?

I like the way you can add attractive backgrounds – either from Microsoft’s collection – or via an image of your own that you upload. When setting up the questions you can add pictures or videos. There are options for the types of questions you want to include: multiple choice, text, or ratings. 


How do you send the completed form to your students?

What is great about it is that you can send this out in a number of ways:

  • You can email it
  • You can just pass on the weblink
  • You can even create a QR code that the viewers can use their QR Readers to open
  • You can embed it in a web page.

How do students respond?

Your students can respond even if they don’t have an O365 account. They can use their web browser or a mobile device.

Your students can fill in the form and submit it. If the questions are all multiple-choice questions the test will be marked automatically and the students will be able to see their results and where they went wrong.   You can also review all the tests submitted back to you and provide further feedback if you wish to. 

How do you collect the data?

The form’s data, such as the test results, can be easily exported to Excel for additional analysis or grading.

This is a great, simple interface to use for polls, quizzes and tests.
